The responsibilities include but are not limited to individualized nursing care, training and supervision. This position is an on call position which is twenty-four (24) hours per day, seven (7) days per week. Provide onsite as well as on-call supervision of staff and non-licensed staffing personnel (i.e. AMAP’s) twenty-four (24) hours per day, seven (7) days per week. DUTIES & RESPONSIBILITIES: • To have a full and complete knowledge of all government (federal, state, city, county, etc.) regulations relating to residential nursing care services for above location(s). • Evaluate each consumer’s need for nursing care as needed. • Review each consumer’s nursing care plan as related to the consumer’s daily needs as per physician’s recommendations. Develop nursing care plans for consumers, as per physician’s recommendations, at least quarterly, or more frequently if needed, for consumers with acute or chronic medical conditions which require treatment or monitoring by health professionals. • Ensure control of communicable diseases. Assessment, appropriate reporting to medical authorities and implementation of appropriate protective and preventative measures, when necessary, of communicable diseases. • Directly and monitor the effects of consumer medications. Review each consumer’s background for potential adverse reactions, allergies, interactions, contraindications, etc. to prescribed medication(s). Perform periodic reviews to ensure consumers are receiving medications as prescribed, that prescriptions are renewed as necessary and that expired and discontinued drugs are disposed of in accordance with medical waster regulations. • Monitor consumer’s weights and diets, at least monthly or more if necessary. • Provide summary reports on consumer’s medical status to staff and develop individualized specific protocols as needed. • Participate in Life Care Plans and other individual treatment planning process and committee meetings. • Participate in in-service training and supervision. • Maintain appropriate medical care and proper records to ensure that annual audits are well within compliance as per OPWDD regulations. • Attend all shift meetings and full staff meetings as needed. • Communicate with other agencies, clinics or hospitals where applicable or necessary as you deem appropriate or otherwise assign LPN and/or AMAP personnel to conduct task. • This position reports directly to the Supervising RN, working proactively to ensure consumer health and wellness that all relevant information is reported directly to the Residence Manager, Coordinator of Residential Services, Associate Executive Director and Chief Executive Officer or designee. • Ensure consumer related issues are reported to team members, residence managers, license practical nurses, physicians, social workers, psychologists, nutritionist, recreations therapist and counselors. • Provide three month, annual evaluation for assigned staff, LPN, during the first year of employment and each year thereafter. • Responsible for teaching AMAP course and re-certifying staff, annually. The agency will provide training for the AMAP Course. • Available to the agency for administering medication in case of shortage of certified staff • To be reimbursed for trips to and from the various locations and meeting where appropriate. To be reimbursed for supplies purchased based on approval from immediate supervisor. • Participate in Self-Survey, QA process as needed. Community Action For Human Services, Inc.
