SHIFT BLOCK 2 : ONE STAFF EACH SHIFT SUNDAY, MONDAY & TUESDAY (11PM-7AM) WEDNESDAY - SATURDAY (11PM-7AM) Job Description Position Summary: The Direct Support Professional (DSP) at Lehigh Valley Companion Care Homes LLC (LVCCH LLC) is a critical member of the care team, responsible for providing direct care and support to individuals residing in the care homes. The DSP plays a vital role in enhancing the quality of life, promoting independence, and ensuring the well-being of the residents while also actively participating in various administrative functions to support the organization's compliance with the waiver program requirements. Responsibilities: Personal Care Assistance: Assist residents with activities of daily living (ADLs) such as bathing, grooming, dressing, and toileting. Support residents in maintaining personal hygiene and grooming standards.Ensure a safe and clean living environment for residents. Medication Management: Administer medications to residents according to prescribed schedules and protocols. Document medication administration accurately and report any concerns or issues to the appropriate supervisor. Monitor medication supply and assist with medication refills as needed. Emotional and Social Support: Foster positive and meaningful relationships with residents, promoting companionship and social engagement. Provide emotional support and encouragement to residents, addressing their emotional and psychological needs. Facilitate and participate in recreational activities, outings, and community integration opportunities. Individualized Care Planning: Implement individualized care plans and support strategies developed by the interdisciplinary team. Document resident progress, behaviors, and outcomes accurately and promptly. Communicate any changes or concerns in the resident's condition to the appropriate supervisor. Behavioral Support: Implement behavioral support strategies and interventions as outlined in individualized behavior support plans. Utilize appropriate de-escalation techniques to manage challenging behaviors effectively. Collaborate with the behavioral specialist and team members to monitor and adjust support strategies as needed. Health Monitoring and Reporting: Observe and monitor residents' health conditions, reporting any changes or concerns to the healthcare team. Document vital signs, incidents, and other health-related information accurately. Documentation and Reporting: Maintain accurate and up-to-date documentation of care provided, incidents, and observations. Complete required reports and documentation according to established procedures and timelines. Ensure confidentiality of resident information and comply with privacy regulations. Communication and Collaboration: Maintain open and effective communication with residents, families, and the care team. Attend team meetings and contribute to the development and review of care plans. Collaborate with other staff members to ensure continuity and consistency of care. Training and Professional Development: Participate in required training programs to enhance knowledge and skills. Stay informed about best practices and industry standards related to direct care. Seek opportunities for professional growth and development. Qualifications: High school diploma or equivalent. Additional relevant certifications are a plus. Previous experience in direct care or a related field is preferred but not required. Knowledge of person-centered care principles and practices. Ability to provide compassionate and respectful care to individuals with diverse needs. Excellent communication and interpersonal skills. Strong observation and problem-solving abilities. Ability to work as part of a team and follow instructions. Willingness to work flexible hours, including weekends and holidays. Physical stamina and ability to lift and assist with transfers when necessary.
