Salary and Benefits Pay-per-visit model: $60 – $67 per point (based on experience; $67 for 20 years) Mileage reimbursement: $0.585 per mile Health insurance – three medical plans through Meritain Health Dental insurance – 50% employer-covered premium Vision insurance – 100% frame allowance at select retailers PTO – 29 days in the first year (with full cash-out option) Retirement – 403(b) with company match Disability – 60% of pay up to $10,000/month Additional benefits – Flexible Spending Accounts, Legal & Identity Protection Plan Responsibilities Provide skilled physical therapy services in a home health setting Conduct assessments including OASIS and ongoing evaluations Create individualized treatment plans in collaboration with interdisciplinary teams One-on-one patient care focused on improving mobility and independence EMR: Kinnser/WellSky preferred but not required Participate in weekend, holiday, or on-call rotations as needed Patient assignments are geographically grouped to reduce travel Supportive team environment with room for career advancement PRN rates available for productivity beyond base expectations Productivity Expectations 30 points per week / 60 points biweekly Start of Care OASIS: 2.5 points Resumption of Care: 1.75 points Recertification: 1.75 points Evaluation visit: 1.25 points Discharge OASIS: 1.5 points Routine visit: 1.0 point Precepting: 0.5 points visit fee Requirements Active Florida PT license At least 1 year of home health experience preferred Current CPR/BLS certification Monday–Friday availability (most visits between 8 AM – 5 PM) Ability to cover Sarasota County and surrounding areas About Us Titan Placement Group is a permanent placement healthcare recruiting firm that bridges the gap between mission-driven healthcare organizations and high-quality candidates.
