Job Description Job Description Children’s Health Home Care Manager Job Description Job Summary: The Children’s Health Home Care Manager is a full-time position housed in a local school district or out in the community that works with youth and their family to develop and maintain a person-centered comprehensive plan for accessing and using services and supports necessary for an individual’s health. Educational Qualifications: - Qualified candidates should have a Bachelor’s degree with a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child & family studies, sociology or speech & hearing; OR a NYS Teacher’s Certificate for which a bachelor’s degree is required; OR NYS licensure & registration as a Registered Nurse & a bachelor’s degree AND two years’ experience in providing direct services to children and families with complex needs and linking these individuals and families to a broad range of services essential to successfully improve home, school, and community engagement (e.g., medical, psychiatric, social, educational, legal, housing, and financial services); OR a Master’s Degree in any of the above programs AND one year of relevant experience. Other position specific qualifications: - Reliable and trustworthy, sound decision-making skills - Demonstrated ability to serve individuals in a professional, welcoming, and efficient manner. - Cooperative, friendly, and helpful attitude with individuals and co-workers. Ability to work closely with other employees and partners to ensure a positive, constructive environment within individual program and overall agency. - Ability to work independently and in a team setting. - Ability to maintain confidentiality regarding the agency’s functions and individuals receiving services. - Must positively promote the agency in the community. - Must be detail oriented. - Must be able to clear NYS Criminal Justice Background Check, Office of Children and Families Background check, Department of Health Background Check, and clear the NYS staff exclusion list, as well as be fingerprinted for the School District. Must clear OIG and HHS-OIG. - Ability to maintain a flexible schedule with availability to work evenings and/or weekends as needed. - Must be willing to learn and self-improve. - Must be willing to travel 50% or more per week. - Must have and maintain reliable transportation including a valid driver’s license, registration, inspection, and insurance. - Job Responsibilities: - Must complete Mandated Reporting, HIPAA, Health Home Netsmart, and Trauma Training with the first week of employment. - Must obtain CANS-NY (Child and Adolescent Needs and Strengths Assessment Certification and complete all NYSDOH training through the Health Commerce (HCS) and Universal Assessments Systems (UAS) within the first 30 days of employment, however, needs to be prioritized. - Suggested to obtain New York State Peer Specialist Certification within 6 months of hire date (this includes Engagement & Outreach, Safety in the Community, Person Centered Planning, Cultural Competency/Awareness, LGTBQI Issues, Meeting Facilitation); TCI certification (Therapeutic Crisis Intervention) - Must obtain New York State Peer Specialist Certification in first 6 months of employment. - Must complete CEU’s to maintain NYS Peer Certification credentials. - TCI refreshers are required every 6 months for six hours to maintain certification. - Must be able to abide by all agency policies and procedures. - Must adhere to professional boundaries with fellow employees, individual’s/families receiving services and partner agency employees. - Attend required trainings and meetings requested by your Direct Supervisor/COO or partner agency. - Must be able to maintain confidentiality of individuals who worked with and agency functions. - Work with identified youth in a school setting alongside the School Based Team to support emotional regulation and employ de-escalation techniques with youth when needed. - Accompany and supervise youth after hours as needed until escalated situation is resolved and/or youth has been connected to appropriate professional. - Must be able to engage and re-engage individuals to ensure positive outcomes. Engagement levels should be 85% or higher monthly. - Must complete intakes per AHI policies and procedures. - Must be in compliance with all tasks as outlined in AHI policies and procedures. - Must completed notes within 24 hours of service delivery. - Must complete and submit all billing requirements by the end of the month. - Be able to provide mental health education with the goal to restore and support environmental stability and avoid more restrictive placements, establish support networks, develop coping strategies toward effective functioning in all relevant environments. - Teach and strengthen daily living skills to assist with self-management and reduction of the effects of psychiatric, emotional and/or physical development or substance challenges. - Identification of personal interests and hobbies within a youth-centered approach - Assess on-going progress toward recovery and functional skill development. - Any other job duties as assigned by the Care Manager’s Supervisor(s).
