Your bilingual skills and compassionate approach could make all the difference in someone's journey to wellness. Make an Impact That Matters As our Bilingual Mental Health Patient Navigator at the Center for Transformative Change, you'll serve as a vital link between our guests facing mental health challenges and the resources they desperately need. Your ability to connect with people, listen with empathy, and guide them through complex healthcare systems will literally change lives. What You'll Do: Become an expert in local mental health resources and build relationships with key providers Meet our guests where they are – physically and emotionally – providing a compassionate presence Assess needs, identify appropriate care options, and create pathways to treatment Break down barriers by providing translation, transportation assistance, and system navigation Maintain confidential records while coordinating care across partner organizations What You Bring: Fluency in Spanish and English At least 2 years of experience in mental health settings Familiarity with medical records/case management systems A compassionate heart with excellent listening and people skills Ability to navigate complex systems and track program outcomes What We Offer: $50,000 annual salary with competitive benefits Monday-Friday schedule, 8:30am-5:00pm Meaningful work within a supportive team environment Opportunity to make a lasting difference in your community Join us in creating transformative change – one connection at a time.
