Position Summary: Case Manager RN Hospice Case Load: Geriatric Territory: King County/ Seattle Area Shift: 8hr Days, M-F 8a-5p Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard) Key Responsibilities: Assessing new patients by gathering information, reviewing diagnoses and analyzing medical test results Developing complete plans of care to address the needs of patients Educating patients about their care plan and health care options and helping them make key decisions Scheduling appointments for doctors’ visits, therapies and other medical services Revising care plans based on patients’ progress and input from physicians and other medical providers Updating patient medical records after each contact Offering emotional support for patients and their families Additional Notes: Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50 miles. Timely communication and documentation required Documentation tools will be provided, and must be returned.
