Nurse Practitioner - Primary Care, Bilingual Cook County IL We are seeking a dedicated Nurse Practitioner to join our primary care outpatient clinic in Cook County IL . This permanent, full-time position focuses on providing exceptional value-based healthcare to Medicare patients, primarily those aged 65 and older. As a vital member of our collaborative care team, you will play a significant role in improving the health and well-being of medically underserved communities.
