Pay Information $3,212 per week About The Position Job Title: Case Manager Profession: Registered Nurse Specialty: Care Coordination/Case Management Duration: 13 weeks Shift: 8-hour Days Hours per Shift: 40 hours per week Experience: Minimum 2 years License: Required Certifications: BLS Must-Have: Candidates must have prior experience with Dignity. Candidates may not have been previously employed by any associated facilities in the past year. Description: The position involves care coordination and case management. The role requires managing 15-20 patient consultations per day. Case managers will deal with various payers, including uninsured and homeless individuals. Responsibilities include completing initial assessments, discharge planning, and utilizing Cerner for documentation. Familiarity with Interqual criteria is required for success in this role. Successful candidates will demonstrate collaboration, teamwork, and effective communication skills. The ability to reprioritize tasks throughout the day is essential. The position involves both utilization review and discharge planning. Weekend work is required every other week, with the possibility of working holidays.
