TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Peoria, Illinois, 61603! Pay Information $2,454 per week About The Position Job Title: Case Manager Profession: RN Specialty: Case Management Duration: 13 weeks Shift: 8 hour days Hours per Shift: 40 hours per week Experience: 2 years of large hospital inpatient Case Management Experience required License: Current RN license Certifications: Case Management certification preferred, BLS - American Heart Must-Have: Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support. Must have previous experience providing care coordination/discharge planning. Description: This position involves assessing patients for both discharge planning and psychosocial needs. You will work with an interdisciplinary team to develop a plan of care for the transition to the next level of care. Overall care coordination for a team of patients will be required. Experience with EPIC is necessary for this role. The job will involve floating to other units/departments, covering approximately 15-20 patients per day. You will be expected to provide care coordination and discharge planning effectively. Daily call responsibilities until 6 PM will be part of the job. Business casual dress code is required, including closed-toe and closed-back comfortable walking shoes. White lab coats are expected as part of the dress code. Training and orientation will include navigating the care management model and identifying patients who need care management support. You will receive one day of in-seat training followed by two days of shadowing. All candidates must understand that this is a Drug-Free Campus requiring a negative drug screen.
