We’re seeking a Registered Nurse Case Manager to join our growing team and oversee a caseload across Mercer County, Middlesex County, Monmouth County and Ocean County . This role is 75% field-based and ideal for an RN who thrives on autonomy, values quality over quantity, and takes pride in delivering personalized, gold-standard care.
