Job Description Job Description The LPN Supervisor is responsible for supervising and implementing an array of services to enrich the lives of people with varied disabilities. Services include but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills. QUALIFICATIONS • Current Licensure by the Commonwealth of Massachusetts as a Practical Nurse • Ability to comprehend and write English at the traditional Grade 12 level • Three years supervisory experience working with people with disabilities, ABI experience preferred but not required • Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes • Acceptable Criminal Offender Record Information (CORI) and fingerprint check SUPERVISOR ESSENTIAL DUTIES • Oversee the day-to-day operation of the program (staff scheduling, medical appointments, community activities, maintenance, vehicles, menu planning, etc.) • Trains, supervises and evaluates residential staff in conjunction with the ADSS/Residential Coordinator II. • Supervise, support, and maintain the safety of individuals. • Provide relevant training or assistance to individuals as identified in their ISP and/or as needed in life routines. • Specific “training” may be in: communication skills, personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP. • Monitor and implement service plans throughout the day including behavioral and therapeutic requirements • Provide case management responsibilities and/or service documentation requirements as assigned and defined by the ADSS/Residential Coordinator II. • Attend required In-services. • Assist individuals with medical appointments as necessary. • Monitor individual’s medical needs as well as doctor’s orders, medication sheets, diets, etc. • Administer and document medication in accordance with applicable regulations. • Ensure required documentation is completed including but not limited to Progress Notes; program data; medication; cleaning checklist; Daily Log/Staff Communication; finances; time slips, etc. • Where necessary, serve as a communication link between residents, family members, guardians and other service providers. • Assist in the management of the program budget and financial record keeping to ensure accuracy and compliance. • Assume various other and related duties as assigned by the ADSS/Residential Coordinator II. LPN ESSENTIAL DUTIES • Confer with physicians, dentists, and therapists as appropriate. • Respond to participant complaints of illness and recommend appropriate action. • Confer with pharmacists regarding medications and their acquisitions as appropriate. • Monitor seizure disorder records. • Administer minor first aid when necessary. • Complete all health-related documentation requirements. • Provide standard First Aid/CPR certification training and/or other assigned trainings to agency employees.
