Travel Hospice Case Manager RN Milwaukee, WI Assignment Length: 13 Weeks Sunbelt Staffing is currently seeking a compassionate and experienced Hospice Case Manager Registered Nurse (RN) for a 13-week travel assignment in Milwaukee, Wisconsin. This role offers a meaningful opportunity to support patients and families during end-of-life care, with a focus on dignity, comfort, and personalized support. Job Responsibilities: Manage a caseload of hospice patients and coordinate individualized care plans Perform in-home or facility-based visits to assess, monitor, and provide nursing care Collaborate with interdisciplinary teams including social workers, chaplains, and aides Educate patients and families on hospice services and symptom management Maintain timely and accurate documentation in the EMR system Participate in on-call rotation as needed Requirements: Active RN license in Wisconsin or compact state Minimum 1-2 years of recent hospice case management experience BLS certification (AHA) required Valid drivers license and reliable transportation Stg communication, time management, and documentation skills Familiarity with EMR systems (HCHB or similar preferred) Why Choose Milwaukee, WI?
