We specialize in complex and high-tech nursing for wound care, intravenous therapies and rehabilitation. We are located in Long Beach (Signal Hill) and service both LA and Orange Counties. All new clinical employees go through an extensive orientation process, that is based on the principals of observing, being observed and independent performance. All new employees are given the option of determining when they are ready for independent performance. We will provide all training needed and facilitate proficiency in tasks for clinical staff. Position Available: Full Time Temporary (4 months, with the possibility of extending further) Los Angeles County Monday – Friday Essential Functions : Performs and oversees admissions (SOC) by evaluating patient in their home or facility. Formulates/oversees plans of care (POC)/Reviews all plans of care for accuracy, and appropriateness of documentations Performs/oversees skilled nursing care in the patient’s home as necessary & Provides teaching to all team members on skills and nursing care including but not limited to documentation. Teaches subordinates and how to and explains the health treatment and teaches the patient and family how to care for the patient. Evaluates patient’s needs and the effectiveness of care and makes changes and adjustments to POC as necessary. Prepares IDT, presents at IDT, generates reports from IDT, facilitates multidisciplinary approach Assigns and Prepares re-certifications Assigns, reviews and prepares discharge summaries Coordinates schedules and delegates tasks to subordinates. Communicates and problem solves with the Medical Director (MD.), DOPCS, Case Management Assistants and other Interdisciplinary Team (IDT) members to insure the patient receives the highest quality of care. Requirements: Minimum Qualifications Bilingual, English and Spanish required. Currently registered with the California State Board of Nurses. Minimum of one (1) year of experience Valid Driver License Car insurance, ability to travel throughout LA County TB test and physical exam within 6 months prior to employment Knowledge of WellSky a plus Haven Health is committed to serving patients and their families with compassion, dignity, and grace.
