Key Responsibilities of the Travel Nurse Provide high-quality care coordination for patients receiving home health services. Assess patient needs, develop care plans, and monitor progress toward health goals. Collaborate closely with physicians, interdisciplinary teams, and family members. Educate patients and caregivers on treatment plans, medications, and self-care strategies. Ensure accurate documentation of patient assessments, care plans, and outcomes. Facilitate transitions of care and referrals to appropriate healthcare services. Requirements of the Travel Nurse Valid Colorado Registered Nurse (RN) license.
