Job Description
UK LEARNING DISABILITY EXPERIENCE IS REQUIRED. As a consistent and competent care professional, upholding the Adult Social Care Code of Conduct, you will:- * Deliver person centred outcome focused care and support to a range of Residents with a Learning Disability, Autism, Physical Disabilities, Mental Health, etc across sites. 1.2 To be a key worker to a number of identified Residents. * To source material, organise, and run therapeutic group activities both in the home, and the Community, this includes daily living & social skills activities. * To provide high standards of physical support/ assistance with personal care as Required, including support individuals with Dysphagia, who will require support with food and drink. * Be a positive part of the team and assist in the running of the home to comply with Care Quality Commission Standards. * To carry out assessments and promote independence. 1.7 To assist with Cleaning tasks, and cooking, and to report repairs as needed. 2. Duties 2.1 Team Actively contribute to the development of the team and service and work to achieve the teams shared objectives and values. To ensure the team operates in a way that keeps Residents at the heart of the service. To communicate effectively and uphold quality assurance and national standards. To work positively with partner organisations. 2.2 Health and safety Understand responsibilities and maintain all standards and systems relating to health and safety. This to include involvement in health and safety check and risk assessments. Effective response to emergencies in accordance with procedures. 2.3 General To assist in the general running of the service in relation to administration, reception, tidying & cleaning, and communication systems. To promote an inclusive atmosphere and actively promote and value diversity. 2.4 Key work Use person centred approaches to support Residents to: * Involve other people who are important including their families and/or carers. * To communicate their needs, and aspirations. * Hold person centred reviews * Develop their outcome focused care plans * To assist with moving to alternative housing, and independent living, and working together with professionals for Best interests. 2.5 Care Planning To undertake with the manager or senior worker initial assessments & care planning of new referrals. To co-ordinate a multi-disciplinary approach and maintain proper records and assessments. To communicate well with families and/or carers. To comply with department standards and legislative requirements in respect of confidentiality 2.6 Care To provide care which consistently achieves Diploma in Social Care standards. To work in compliance with local and department procedures. To comply with the Adult Social Care Code of Conduct. To protect vulnerable Residents from abuse and comply with department standards for reporting abuse. 2.7 Safeguarding To be observant for signs of potential abuse and to respond to disclosure of abuse. To follow safeguarding procedures for reporting and recording of abuse. 2.8 Administration To competently use I.T. To access client database, communicate via email, and create professional reports, which comply with the departments standards for recording. To ensure all monies are managed within Departmental guidelines e.g. petty cash and Residents finances
