Job Details – Pediatric Case Management RN Location: Webster, NY 14580 (Greater Rochester, New York area) Healthcare Setting: Pediatric Home Care (CHHA/OASIS) Employment Type: Travel RN, On-Site Contract Duration: 13 weeks (11/17/2025 – 2/14/2026) Schedule: 32 hours/week, Friday – Sunday, 8:00am – 4:30pm Training: First two weeks Monday–Friday, 8:00am – 4:30pm Shift Duration: 8-hour shifts Estimated Weekly Salary: $2250 - $2310 Qualifications – Pediatric Home Care RN Active New York State Registered Nurse (RN) license Minimum 1 year of clinical nursing experience in a hospital or acute care setting Certified Home Health Agency (CHHA) and OASIS experience required Pediatric nursing experience strongly preferred BLS certification (any provider accepted) Experience with EPIC or Netsmart EMR systems preferred Telemetry experience required Valid NYS driver’s license and current auto insurance Ability to travel throughout the Webster, NY service area year-round Strong interviewing, assessment, and communication skills Proficiency with Microsoft Office Suite and electronic medical records Key Responsibilities – Case Management RN Develop and implement individualized care plans for pediatric home care patients, collaborating with physicians, families, and the clinical healthcare team Complete OASIS assessments to accurately document clinical, functional, and social needs Maintain timely and accurate clinical records to ensure compliance with New York State and federal regulations Refer pediatric patients to appropriate healthcare systems and community resources Communicate assessment outcomes to the healthcare team for coordinated follow-up care Demonstrate professionalism, patience, and flexibility in all patient and family interactions Resolve issues at the point of contact and maintain productivity standards Adhere to professional standards for home health nursing visits Work independently, adapt to change, and manage multiple assignments efficiently Apply now to join our pediatric home care team in Webster, NY and make a difference in the lives of children and families!
