Support Worker / Mentor

Life Purpose Australia is a trusted NDIS provider based in Wembley, WA, supporting individuals to build independence, confidence, and community connections. We are currently seeking a female Support Worker/Mentor who is passionate about empowering young people and adults living with disabilities. In this role, you’ll provide 1:1 community access and mentoring support, helping participants achieve their goals through engaging activities and positive relationships. You’ll also play an active role in our social programs designed to promote inclusion, skill-building, and wellbeing. Duties include: Supporting participants to engage in community and recreational activities Providing assistance with daily living and social participation Encouraging independence and personal growth Maintaining accurate shift notes and communication via ShiftCare Requirements: NDIS Worker Screening Check Working With Children Check First Aid & CPR Certificate Current driver’s licence & reliable car Experience supporting people with disabilities (preferred) Saturday availability essential
